Physician Assistant – Full-Time Position Multi-Specialty Private Physician Group

We are a multi-specialty private physician group seeking a dedicated and motivated Physician Assistant to join our growing medical team on a full-time basis.

Qualifications:

Active New York State Physician Assistant license required

Family Medicine, Internal Medicine, Geriatrics, Rehabilitation, or Post-Acute Care experience preferred

Previous experience in skilled nursing, rehabilitation, long-term care, or assisted living is a plus

Strong communication, clinical judgment, and organizational skills

Ability to work collaboratively with physicians, nurse practitioners, nursing staff, and facility teams

Willing to train the right candidate

Responsibilities:

Manage acute, chronic, medical conditions under physician supervision

Perform patient evaluations, follow-ups, and clinical assessments

Coordinate patient care with attending physicians, families, nursing staff, and facility teams

Communicate regularly with physicians regarding changes in patient status and treatment plans

Support the medical team in providing high-quality, patient-centered care

Participate in care coordination, documentation, and clinical follow-up

Help ensure continuity of care across post-acute and long-term care settings
