Home Health RN Case Manager

RN Case Manager – Home Health Responsibilities include planning, coordinating, and delivering quality home health care and services in compliance with company and regulatory standards. Essential Activities and Tasks Care Management and Service Delivery – 70% Provides ongoing assessment and nursing care to patients in homes and healthcare facilities. Ensures accurate evaluation of patient needs and develops and implements a comprehensive care plan. Identifies when patients need or qualify for additional services and notifies appropriate team member or leader. Follows nursing practice standards and agency protocols for home health. Provides quality plan and symptom management for patient comfort. Communicates with other health care providers regarding changes in condition and needs. Assesses patient and caregiver needs and provides education. Commits to high quality customer service for internal and external customers. Oversees implementation of the patient’s plan of care and evaluates effectiveness continuously. Quality and Compliance Management – 30% Audits and maintains current and accurate patient records via laptop and paper/chart documentation per policies and guidelines. Participates in quality improvement projects as needed. Maintains knowledge of regulatory agencies, accrediting bodies, and third‑party payers. Performs all other duties as assigned. Qualifications Current unencumbered RN license in the state of Ohio. One year experience in acute care or medical‑surgical nursing. Experience in a Medicare‑certified home health agency preferred. Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint) and the internet. Proficiency with electronic medical records (EMR) preferred. Additional Requirements Must read, write, speak, and understand the English language. Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards. Working Conditions and Special Requirements Sitting – up to 4 hrs/day; Standing – up to 8 hrs/day; Walking – up to 6 hrs/day. Lifting, transferring, pushing or pulling patients or equipment/supplies – up to 50 lbs. Driving – up to 6 hrs/day. Work weekends, evenings, and holidays – required as scheduled. On‑call availability – 24/7 for emergencies. Subject to falls, burns from equipment, odors, cuts, various disease processes.
