Looking for a change of scenery and a chance to help others. Join a travel Physical Therapist role in Dallas, GA providing home health services where you will help patients recover from injuries and manage chronic conditions while experiencing life in a new location. You will perform patient evaluations and provide therapeutic services according to physicians' orders. You will supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Reliable transportation, a current driver’s license, and agency-required liability insurance are required for home visits.

Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accrediting body.
• Current, unencumbered state license to practice physical therapy in the state of assignment (Georgia) is required to start the assignment.
• Current CPR certification.
• Experience with home health settings or providing skilled physical therapy in patients’ homes preferred.
• Must have reliable transportation, current driver’s license, and agency-required liability insurance for home visits.
• Familiarity with electronic documentation, email, basic computer navigation, and online training/HR systems preferred.
• Ability to supervise and evaluate PTAs and home health aides as assigned. Preferred: 2–3 years of physical therapy experience.

Preferred Qualifications

• 2–3 years of physical therapy experience preferred.
• Experience coordinating and updating plans of care, performing home environment evaluations, and participating in discharge planning.
• Prior experience with telecommunication supervision and periodic on-site assessments in home health settings preferred.

Assignment Details

• Shift name: Evenings.
• Schedule: 5x8.
• Must be able to perform home visits and travel locally within the service area as required.

Impacting Patient Care

Provide skilled physical therapy in accordance with physicians’ plans of care. Duties include providing treatment as ordered, assisting in evaluation of function, developing and updating plans of treatment, documenting and reporting patient condition, instructing family and agency personnel when appropriate, and participating in in-service education. Provide supervision of PTAs and coordination of discharge planning to ensure appropriate, timely care.
