Job Description


RATE caps for Submission(STRICTLY FOLLOW)

Malpractice - Must be included in rates.

Orientation - $312.50-330 - half the hourly/callback rate, kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"

Schedule: M-F 8a-5p with weekend call from Friday 5pm to Monday 8am (remote call only).

Weekend shifts start Friday @5pm through Monday ending at 8am.

Weekday shifts are clinic Monday-Friday (8am-5pm) with Night Call Monday-Thursday (5pm-8am)


Weekday coverage needed

• August 31- Clinic CLOSED 8/31 (need remote call only)
• Nov 26 - clinic closed, remote call only
• Nov 27 - clinic closed, remote call only (another provider on call starting at 5pm)
• Dec 25 - clinic closed, remote call only (another provider on call starting at 5pm)
• Weekday shifts are clinic Monday-Friday (8am-5pm) + Night Call Monday-Thursday (5pm-8am)

• Board Certification - Board Certified or Board Eligible in Oncology or Hematology [REQUIRED]
• CA License [REQUIRED]
• Fellowship [REQUIRED]
• Other Certifications - DEA [REQUIRED] . ACLS only [REQUIRED]if requesting Moderate Sedation Privileges ( PALS for Pedicatric Patients)
• Must list dates of availability [REQUIRED]
