What We Offer Full-Time Competitive Compensation Medical, Dental & Vision Insurance PTO & Paid Holidays Aflac Options Flexible Daily Scheduling Company Tablet Supportive Clinical Leadership Professional Growth Opportunities PRN Competitive PRN Rates Flexible Scheduling Build Your Caseload Around Your Availability Consistent Patient Opportunities Company Tablet Supportive Clinical Team What You'll Do Complete skilled nursing assessments and home visits Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned Develop and update individualized plans of care Provide wound care, medication management, and disease education Monitor patient conditions and report significant changes Educate patients and caregivers Coordinate care with physicians, therapists, and other team members Complete accurate and timely documentation using WellSky Help prevent unnecessary hospitalizations and support patients safely at home Qualifications Active Washington, DC Registered Nurse License Required Graduate of an accredited nursing program Current CPR/BLS Certification Strong clinical assessment and communication skills Dependable transportation to patient visits Ability to work independently in patients' homes Home Health and OASIS Experience Preferred WellSky experience is a plus Why Human Touch?
