Job Description

The Patient Care Assistant plays a crucial role in supporting patients and healthcare teams in home health settings. This position involves assisting with patient care tasks, monitoring vital signs, documenting medical information, and collaborating closely with healthcare professionals during 8-hour shifts to ensure high-quality patient support.

Responsibilities

Provide direct support and assistance to patients with daily activities
Monitor and record vital signs accurately
Document medical information and patient progress clearly
Collaborate effectively with healthcare team members to coordinate care

Required Qualifications

Minimum 1 year of experience in patient care
High school diploma or equivalent
Valid CPR certification

Preferred Qualifications

Knowledge of basic patient care techniques
Skill in measuring and interpreting vital signs
Strong communication skills
Demonstrated teamwork ability
Empathy and compassionate patient interaction
