Job Description
Title: Home Health Occupational Therapist

Schedule:
- Shift: 8-hour days
- Weekend requirement: No

Assignment Details:
- Visit volume: Up to 6 visits per day
- Guaranteed hours: May call off up to one shift per pay period

Requirements:
- Minimum 1 year of skilled home health OT experience (preferred)
- Required certification: BCLS
- OASIS experience required

Roles & Responsibilities:
- Provide occupational therapy services to adult and geriatric patients in a home health setting.
- Assess patients' needs and develop individualized treatment plans to improve activities of daily living (ADLs).
- Document patient progress and adjust treatment plans as necessary, including OASIS documentation.
- Collaborate with healthcare professionals to coordinate care and ensure the best patient outcomes.

Compliance / Notes:
- References: 2 references covering 1 year within the last 3 years required.
- Charting systems: EPIC Remote Client for Home Health and Rover app.
- Dress code: Business casual preferred; scrubs allowed (no jeans).
- Parking: Free.
