Overview Physical Therapist, Geriatrics — Polk County, NC MISSION-DRIVEN non-profit Continuing Care Retirement Community focused on high-quality geriatric care in a resort-style setting. This role emphasizes knowing residents deeply and delivering individualized care rather than rushing through quotas. Responsibilities 65–75% productivity expectation: focus on care rather than counting units; 85% outpatient caseload with active, engaged residents Salaried model with stable pay when a patient cancels; no on-call, no weekends. Monday–Friday, 40 hours/week Follow residents through their continuum of care (Independent Living, Assisted Living, Skilled Nursing) rather than handing them off; provide individualized care in each session Collaborate with the in-house therapy team; opportunities to shape programming and grow clinical skills on-site (vestibular rehab, chronic pain, pelvic health, and more); CEUs included This campus offers a full continuum of care, which supports continuity in resident care and reduces handoffs.
