Licensed Master Social Worker (LMSW)
Location: New York, NY
Duration: Contract
Shift: Day Shift (9:00 AM – 5:00 PM)
Schedule: 5x7s
"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker to provide psychosocial assessments, supportive therapy, and discharge planning services in an inpatient medicine/surgery setting. Role focuses on patient support, care coordination, and referral management.
Key Responsibilities- Conduct psychosocial assessments for inpatient patients
- Provide supportive counseling and therapy
- Perform discharge planning and care coordination
- Make appropriate patient referrals to community resources
- Collaborate with interdisciplinary healthcare teams
- Document assessments and interventions in EMR
- Support inpatient medicine/surgery unit operations
- Ensure compliance with hospital social work standards
- Master’s Degree in Social Work (MSW)
- Active LMSW license (NYSED)
- Minimum 1 year LMSW experience
- Strong communication and documentation skills
- Experience in inpatient hospital setting preferred
