Occupational Therapist - Sign On

Role: Occupational Therapist in Home Health responsible for patient assessments, evaluations, and care planning related to functional status, daily activities, fine motor skills, home safety, and adaptive equipment. Key Responsibilities Provide services within state practice laws, following evidence-based techniques and collaborating with the healthcare team. Evaluate patients' functional abilities and health status, and assist physicians in developing therapy plans. Monitor and document patient responses to treatment and report changes to supervising nurses and physicians. Conduct diagnostic assessments to determine functional levels and prognosis. Qualifications Current Occupational Therapy licensure in the practicing state. CPR certification, valid driver's license, vehicle insurance, or access to reliable transportation. Ability to work effectively in diverse home environments regardless of client background. Preferred Skills Ability to manage multiple tasks independently. Strong communication, organizational, and writing skills.
