Compassionate Hospice RN Case Manager

Gentiva is seeking a Hospice RN / RN Case Manager in Texas to lead patient care at home or in community settings. You’ll be the primary contact for patients and families, delivering skilled nursing care and coordinating with an interdisciplinary team. This role emphasizes compassionate support, caregiver education, accurate documentation, and adherence to regulatory standards.
