Hospice RN Case ManagerHospice RN Case Manager is responsible for the planning, coordinating, providing and documenting of care of terminally ill patients from day of admission through day of discharge.MAJOR TASKS, DUTIES AND RESPONSIBILITIES:Keeps Clinical Director updated on patient statusDevelops, evaluates, reviews and implements a plan of care for each patientResponsible for the performance of nursing in all areas to facilitate effective, efficient patient careAssures quality care by reviewing and assessing each serviceResponds to the physical, mental, social and emotional needs of terminally ill patients and their familiesResponsible for supervising nursing work in cooperation with Director of Hospice ServicesProvides overall planning, direction, coordination, management and supervision of all nursing and professional services in conjunction with the Director of Hospice ServicesIs an active member of the Interdisciplinary Team and attends Interdisciplinary Team meetings as scheduledParticipates in the On-Call rotation to provide 24/7 coverage for the hospice populationQUALIFICATIONS:Nursing Degree from an accredited college and current RN license in the State of Massachusetts.Minimum of 2 years' experience in a clinical settingExperience in program development, implementation and managementExperience in community educationExcellent organizational skillsAbility to work with all members of the Interdisciplinary TeamMeets federal and state licensing requirements.Ability to communicate in English effectively through verbal and written means.Ability to work under stress and in emergency situations.Understands principles of growth and development for the entire lifespan with the ability to provide care appropriate for adult and geriatric patients as appropriate to the patient population.If you're looking for a career that's transformative for you and those you serve, then you've come to the right place.
