Develop and implement individualized treatment plans to improve ADLs, functional mobility, and safety with adaptive equipment as needed. Deliver direct therapies and group activities aimed at maximizing independence and quality of life. Monitor patient progress, adjust interventions, and document outcomes in the medical record. Collaborate with nursing, therapy, and social work teams to support discharge planning and continuity of care. Ensure compliance with regulatory standards and safety guidelines; participate in care conferences as part of the interdisciplinary team.
