50/50 mix of pediatric and adult patients
Provider must commit to a minimum 3-month assignment (with option to extend)
Chart closure required within 72 hours

Tuesday Friday 7:00 AM 6:00 PM
Coverage Type: Scheduled Clinical Hours Only

No hospital call

Candidate Requirements

Active California License REQUIRED
Active DEA prior to start REQUIRED
Board Certification NOT required (Board Eligible accepted)
Must be willing to work at least 3 months No exceptions

Credentialing Immunization Requirements:
Flu Vaccine
TB Status / Chest X-ray within 90 days
MMR Immunization or Positive Titer
Hepatitis B Immunization or Positive Titer
Varicella Immunization or Positive Titer
Tdap within last 10 years
