Highlights Location: Iselin, NJ Facility: Hackensack Meridian Health Schedule: Full-time What You'll Do Provide professional nursing care to patients in their home. Assess, develop, implement, and evaluate home nursing care needs. Ensure compliance with physician orders and documentation. Demonstrate holistic care for patients and their significant others. Complete nursing assessments and medication reconciliations. Collaborate with physicians on the Plan of Care. Act as a liaison among physicians, management, and healthcare team members. Supervise para-professional home health aide staff. Maintain awareness of new standards and incorporate them into practice. Complete routine assignments independently and meet productivity requirements.
