The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function, and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

KEY DUTIES:

• Effectively screens/evaluates patients and/or ADL physical functioning disorders and develops appropriate plans of care following all regulatory and clinical practice standards

• Utilizes standardized assessments and evidence-based practice to support clinical interventions

• Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements

• Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices

• Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements

• Evaluates treatment outcomes for effectiveness and modifies services in relation to patient evaluation and referrals are made in relation to outcomes

• Provides consultation and counseling to patients, families, caregivers, and other service providers related to the patients' physical disorders

• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans

• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements

• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy

• Accurately records treatment minutes for services provided per all regulatory and clinical requirements

• Participation in required meetings, including patient care conferences, utilization review meetings, rehabilitation meetings, etc.

• Adherence to NBCOT/AOTA and State Required practice acts, code of conduct, ethics during professional practice as an Occupational Therapist

• Adherence to Company required policy and procedures and code of conduct

REQUIREMENTS / EDUCATION / EXPERIENCE:

• Bachelor's degree or higher in Occupational Therapy or related field

• Active state license

• 2+ years of experience in a skilled nursing facility environment
