Physical Therapist

We are seeking a licensed Physical Therapist to deliver personalized, one-on-one care to older adults in their homes and senior living communities. Key Responsibilities Include Performing evaluations and developing individualized treatment plans to enhance mobility, independence, and overall well-being. Providing evidence-based therapy sessions and documenting care using electronic medical records. Collaborating with interdisciplinary teams, including PTs, OTs, SLPs, and fitness specialists. Educating patients, caregivers, and staff to promote safety and aging-in-place strategies. Utilizing telehealth and virtual care options to increase access and flexibility. Qualifications And Skills Valid Physical Therapy license and degree from an accredited program. Proficiency with basic computer and EMR systems. Current CPR certification. Physical Requirements Include Moderate physical effort, including lifting up to 50 lbs and assisting with patient transfers. Driving to patient locations and performing therapeutic activities.
