Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Oregon state license required to start the assignment.
• Current BLS certification from AHA or ARC required.
• Experience in outpatient rehabilitation required; candidate must be comfortable with a caseload that is primarily outpatient with some inpatient coverage (approximately 20% inpatient).
• Experience with Epic electronic medical record preferred.
• Availability to provide occasional Saturday coverage as needed (typically 2 Saturdays within a 13-week contract) preferred.
• Prior experience working in skilled nursing, outpatient rehab, or similar post-acute settings preferred.

Assignment Details

• Facility type: Skilled Nursing Facility with primarily outpatient rehab responsibilities.
• Shift: Days.
• Shift hours: 07:30 to 18:00.
• Schedule pattern: Four 10-hour days per week.
• Occasional Saturday coverage required (typically two Saturdays during the assignment).
• Assignment emphasis: Primarily outpatient caseload with approximately 20% inpatient responsibilities.
