RN - Clinical Care Leader - Bethany - FT - Day Shift

Overview The Clinical Care Leader (CCL) is responsible for utilizing the nursing process to provide individualized nursing care in the home setting. The role collaborates with inter‑professional colleagues to plan, implement, and evaluate care, documents resident history, and serves in leadership roles on departmental activities. Shift & Compensation Day shift (Monday–Friday), 40 hours per week. Compensation includes a salary range of $41.00 – $55.00 per hour plus a $10,000 sign‑on bonus. Additional incentives: $1.25 per hour for weekend work and $1.25 per hour for hours worked 6:00pm–6:00am. Responsibilities Utilize the nursing process (assessment, diagnosis, planning, implementation, evaluation) for individualized care. Collaborate with inter‑professional staff to plan, implement, and evaluate resident care. Document resident history, symptoms, medications, and care provided in compliance with standards and regulations. Use technology to enhance quality resident care and collaborate with team members. Delegate to and supervise unlicensed personnel. Assume overall nursing accountability for resident assignments and lead departmental initiatives. Demonstrate competency within nursing expertise and apply age and population specific standards. Communicate effectively with peers, residents, and families, negotiate and resolve conflicts. Apply clinical reasoning and critical thinking to analyze data and respond to changing situations. Coordinate care, conduct resident assessments, educate residents, triage, and perform other nursing interventions. Operate within the scope of the Nurse Practice Act and state administrative rules. Qualifications Bachelor’s degree in Nursing preferred; graduation from an accredited nursing program (CCNE, ACEN, or NLN CNEA). Minimum of two years’ clinical experience preferred. Unencumbered RN license in Minnesota or multistate licensure in a Nurse Licensure Compact state. Valid driver’s license (may be required based on location). Basic Life Support (BLS) certification required. Resident Assessment Instrument (RAI) process experience preferred. Must obtain and maintain required department‑specific competencies and certifications.
