Job Title: RN - Case Manager
Shift Details: 08:00 AM - 04:00 PM, 5 shifts per week
Scheduled Hours: 40
Start Date: ASAP
End Date: 11/02/2026
Duration: 13 weeks
Job Description: Day 5x8-Hour (08:00 - 16:30)
Experience must be current acute hospital experience, will not accept candidates that have not worked in the role in the last 6 months. Hardstop requirements include knowledge of Interqual and Epic.
Weekend Coverage: True, at least 2 in a 4 week schedule to meet department needs
Patient Age Groups: Adults, Geriatrics
Daily Census: Case load up to 20-22 patients
Interdisciplinary Support: Social Services
Patient Diagnoses: Experience must be current acute hospital experience, will not accept candidates that have not worked in the role in the last 6 months. Hardstop requirements include knowledge of Interqual and Epic.
Special Procedures/Unit Details: Admission Criteria (InterQual)(ED only) Experience with HMOs, IPAs, etc. Experience with Assessing patients, Care Coordination, and Discharge Planning 2-year acute care Hospital inpatient Case Manager experience performing Care Coordination and Admission Reviews
Computerized documentation required.
