Pay Range: $102107 - $135000/annum. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Lead and oversee daily Case Management department operations.
- Supervise staffing, team development, onboarding, and staff competencies.
- Ensure effective care coordination, discharge planning, and timely documentation.
- Monitor departmental performance and implement quality improvement initiatives.
- Ensure compliance with organizational policies and regulatory requirements.
- Collaborate with hospital leadership, including the CFO, CMO, CNO, and Division Case Management leadership.
- Drive productivity, resource utilization, and patient care outcomes.
Key Requirements and Technology Experience:
- Minimum 2 years of recent acute care Case Management leadership experience within the last year.
- Bachelor's degree required.
- Active RN or LCSW credentials; RN preferred.
- Master's degree in Nursing, Healthcare Administration, or Business Administration preferred.
- Strong leadership, communication, team-building, and change management skills.
