Greenfield Rehabilitation Agency is seeking a PRN Speech-Language Pathologist (SLP) to support therapy services at our highly respected senior living communities in Sauk City, Verona and/or Middleton. This is an excellent opportunity for a compassionate and skilled clinician looking for flexible, as-needed hours in a supportive, therapist-led environment.

What You'll Do:

• Evaluate and treat adult and geriatric residents with communication, cognitive, and swallowing disorders
• Develop and implement individualized, patient-centered treatment plans
• Collaborate with interdisciplinary team members to enhance quality of life and function
• Accurately document services per company and regulatory standards

What You Need:

• Current Wisconsin Speech-Language Pathology license and ASHA CCCs
• Excellent communication and documentation skills
• A passion for working with older adults in skilled nursing and senior living settings
• Professionalism, flexibility, and a team-oriented mindset
