RN - Case Manager

Job OpportunityWe are seeking dedicated healthcare professionals for an opportunity at an acute care facility. This position is expected to start as soon as possible and requires floating between various units as needed.Schedule and Work EnvironmentShift: Day shift, 5x8-hour format (08:00 AM - 04:30 PM)Workweek: Variable 5-day workweek with operations occurring 7 days per week.Days Off: Days off will vary and may not be consecutive, although efforts will be made to schedule consecutive days off when possible.RequirementsExperience: A minimum of 1 year of acute care experience is required. Candidates without this experience will be redirected.Caseload: The role involves managing a caseload of 20-23 patients.Licensure: A valid California state license is required at the time of submission.Certifications: Must possess BLS certification. LCSW or MSW is preferred.Travel Experience: Previous travel experience is required.Skills and ResponsibilitiesThe ideal candidate will have knowledge in the following areas:Care management and discharge planningCoordination of patient services and comprehensive assessmentsFacilitating communication between patients, caregivers, and healthcare teamsExperience with EMR systems, primarily EpicKnowledge in patient education, crisis intervention, and advance directivesPatient Care DetailsPatient Age Groups: Adults and GeriatricsDaily Patient Census: Approximately 25 patientsUnit Environment: The care management department consists of RN's and MSW/LCSW's who perform collaborative discharge planning.Other InformationOrientation Hours: 40 hoursWeekend Requirements: Variable schedule; coverage may be necessary.Floating Expectations: Candidates may be required to float to nearby facilities as needed.Application ProcessIf you meet the above requirements and are ready to join a dynamic team, please apply to this opportunity today!
