Hours: 36 hours/week (guaranteed)
Physical Therapist needed for a Skilled Nursing Facility in Auburn, California providing rehabilitative care to post-acute and long-term care patients. This role focuses on restoring mobility, strength, and functional independence in a structured SNF setting.
ResponsibilitiesEvaluate patients and develop individualized plans of care
Provide skilled physical therapy interventions
Monitor patient progress and modify treatment plans as needed
Collaborate with interdisciplinary care teams
Complete timely and accurate documentation per facility guidelines
Active California Physical Therapist license
Skilled Nursing Facility or post-acute care experience preferred
