Work Environment

In this role, you will primarily work onsite, visiting patients in their homes across Freehold and surrounding areas. Key Responsibilities

• Conduct comprehensive evaluations that include patient history, specific assessments, and the formulation of individualized treatment plans, ensuring meticulous documentation of results.
• Effectively communicate evaluation findings and treatment strategies to patients and their families, actively involving them in goal-setting to foster ownership of their rehabilitation journey.
• Implement tailored treatment protocols based on assessment outcomes, adapting interventions as necessary to align with patients' evolving needs and healthcare trends.
• Collaborate with interdisciplinary healthcare professionals to provide holistic care; recognize and address changes in patient conditions that necessitate modifications to the care plan.
• Manage discharge planning and follow-up processes, ensuring a comprehensive understanding of patient care pathways and maintaining accurate and timely documentation of all patient interactions and treatment sessions.
• Demonstrate effective time management skills by balancing patient caseloads, adhering to a structured schedule, and participating in marketing initiatives to promote our services within the community.
• Contribute to the department's objectives by engaging in compliance with healthcare regulations and enhancing service delivery standards.
• Commit to professional development by pursuing continuing education opportunities and serving as a clinical instructor to students and new staff.
• Ensure a positive patient experience by being adaptable and responsive to individual patient needs while actively participating in our Access NRH initiatives to improve community engagement.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid DPT license in the State of New Jersey is essential.
• Current American Heart Association's BLS for Healthcare Providers CPR certification is necessary.

Preferred Experience

• While prior clinical experience is not mandatory, a background in home health care settings or previous internships during academic training will be advantageous.
