Cancellation Policy: For Agency or Client PRIOR to start - can cancel within 21 days with NO penalty. If during 21 days prior to start than cancellation fee equal to one (1) week of the Assigned Clinical Professional's Guaranteed Minimum Hours multiplied by the Assigned Clinical Professional's billing rate. Client may end the engagement of a Clinical Assigned Professional whose engagement has already begun by paying a cancellation fee equal to the Clinical Assigned Professional's billing rate as set forth in EXHIBIT F, multiplied by two (2) weeks of the Clinical Assigned Professional's Guaranteed Minimum Hours less the number of hours worked by the Clinical Assigned Professional after receipt of written notice of cancellation. Once an Assigned Professional has begun his or her assignment with Client, if such Assigned Professional does not fulfill the contract terms, agency will pay a cancellation fee equal to the Assigned Professional's bill rate multiplied by one (1) week of guaranteed minimum hours. Any request made by Agency to cancel an assignment will only be recognized by SimpliFi (or deemed received by Client) as of the date upon which written notification of such cancellation is received by SimpliFi. Client is limited to thirty-six (36) hours per thirteen (13) week assignment for Low Census Call Off. Low Census Call Off if defined as terminating a specific shift for an Assigned Professional due to low census at the Client facility. Client must attempt to provide at least two (2) hours' notice prior to any Low Census Call Off. May hire perm any professional that has completed at least 468 hours of work in a thirteen (13) week assignment at no fee.
