Minimum Qualifications

• Bachelor's degree in social services, counseling, or psychology required. Master's degree preferred.
• Experience or education in grief or loss counseling required.
• Healthcare Provider Level BLS required.
• Experience working in hospice, home health, or related community settings preferred.
• Current state licensure or credentials as required for the role.

Preferred Qualifications

• Master's degree in social work (MSW) from an accredited program preferred.
• Previous experience coordinating bereavement programs or hospice social work preferred.

Assignment Details

• Shift hours: 0800-1630.
• Occasional weekends required.
• May include work in patient homes, skilled nursing facilities, assisted living, and other community settings.
