This role offers the chance to work in a close-knit community, providing essential care to residents in a long-term care/skilled nursing facility.

As a Licensed Practical Nurse, you will play a crucial role in ensuring the well-being of residents by:

• Observing residents continuously and reporting changes in mental and physical health to the Supervisor
• Receiving, transcribing, and executing physician's orders accurately
• Administering medications and treatments and documenting them in a computerized system
• Supervising LNAs and assisting with nursing care as needed
• Accompanying physicians on rounds to address medical needs
• Performing wound care and managing gastrostomy/jejunostomy feedings
• Ensuring infection control practices and a hazard-free environment
• Communicating effectively with residents' families and addressing their concerns
• Managing the assigned unit during shifts and ensuring compliance with regulations

Requirements for this role include:

• Need 3 years experience in setting
• IV certification (or willingness to obtain during onboarding)
• COVID-19 vaccine (declinations/exemptions not accepted)
• Basic Life Support (BLS) certification from AHA
• Active New Hampshire state license
• Willingness to float where experienced
• Previous travel and rural/small facility experience highly preferred
