Hospice RN Clinical Liaison: Admissions & Care Coordination

Gentiva Health Services, Inc. is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and referral processes for hospice services. You will build relationships with referral sources and coordinate admissions while educating families about hospice care. This role requires a Licensed Nurse with extensive hospice experience.
