Role Overview: This position involves leading the implementation and ongoing monitoring of personalized care plans to ensure high-quality, tailored patient treatment. Key Responsibilities Coordinate care across multiple clinical disciplines, serving as the central communication hub to align the healthcare team for optimal patient outcomes. Assess and facilitate patient transitions from admission to discharge, collaborating with team members to ensure safe and seamless recovery journeys. Communicate critical patient condition changes to the medical team in real-time, providing essential clinical insights for prompt intervention. Qualifications & Skills Current licensure as a Registered Nurse, with licensure obtained from the Tennessee Board of Nursing or a multi-state license recognized in 'Compact State'. Basic Life Support (BLS) certification obtained within 30 days of hire, accepted from American Heart Association or American Red Cross. Diploma from an accredited nursing school or equivalent professional licensure at the time of employment.
