Job Summary:
We are seeking a skilled and compassionate Home Health Registered Nurse (RN) to join our team in Lowville, NY. The ideal candidate will provide quality in-home nursing care, coordinate with healthcare providers, and ensure the best outcomes for patients in their home environment.

Responsibilities:

• Deliver comprehensive patient care in the home health setting.

• Assess, plan, and implement individualized patient care plans.

• Administer medications and treatments as prescribed.

• Educate patients and families on care techniques, medication, and follow-up procedures.

• Coordinate with physicians, therapists, and other healthcare professionals.

• Maintain accurate and up-to-date documentation of all care activities.

• Participate in call rotation and respond to patient needs during call hours.

Requirements

Requirements:

• Active and unencumbered RN license in New York State (must be currently active).

• Minimum of 1 year of recent home health experience preferred.

• BLS certification required.

• Strong communication, organizational, and critical-thinking skills.

• Ability to work independently and manage multiple cases efficiently.
