Windsor Home Health company, a division of PREMIER THERAPY, believes in the importance of helping people to live independent and dignified lives.At Windsor Home Health, every individual employee contributes directly to the success of our mission, which is to enrich our patients' lives.We are looking for a Home Health Licensed Practical Nurse (LPN) to join our team Full-time and help our patients remain comfortable, at home.This is a great opportunity for a Licensed Practical Nurse with home health experience who loves the life-changing impact your nursing work has on your patients!You Will:Provide nursing care under the direction of the Home Health Registered Nurse and the established Plan of Care to achieve positive patient outcomes.Communicate with family members and other members of the patient's multi-disciplinary teamWhat We Need from You:A commitment to helping our home health patients remain comfortable, at home.Proof of graduating from an accredited school of practical nursing.Current LPN license from the State of Ohio.Previous experience in home health care setting preferred.Superior organizational and time management skills.Reliable transportation, with a valid driver's license, insurance and registration to travel in assigned territories.Current CPR certification.Demonstrated compassion, empathy and strong customer service skills.A completed and successful TB test, physical examination and a criminal background check.What We Offer to You:Your voice matters!
