Specialty Ochsner Abrom Kaplan Memorial Hospital-AP-Physician-0001 Location 1310 West 7th Street, Kaplan, Louisiana 70548 State LA Position Overview Patient Volume: 6800 Annual ED Volume Coverage Model: 24-hour physician coverage Number of Beds: 20 hospital beds and 6 ED beds Acuity: Level 4 EMR: EPIC Moonlighting: EM residents only Schedule Days, Nights Requirements License – Active unrestricted LA Medical License, LA State Controlled Substance, DEA with LA address & schedules 2, 2N, 3, 3N, 4 & 5 and Supervising Physician License with Louisiana State Board of Medical Examiners Board Certification – Board Certified/Eligible in EM/FM/IM preferred Merit Badges – ACLS, ATLS, PALS required if not EM trained Immunization – Flu (within current flu season) | TB (past 12 months) or Chest X‑Ray with positive TB Vaccinations Case Log – Documentation of case logs for past 2 years CME/CEU – Proof of completed CME's for the last two years.
