LVN Position in Rancho Bernardo, CA

Star Nursing has urgent per diem needs for LVNs in Rancho Bernardo, CA. Location: Rancho Bernardo, San Diego, CA. Start Date: ASAP. Shift Schedule: Monday to Friday, 8:30 AM–5:30 PM. Openings: 2 LVNs. Contract Length: Full Time / Per Diem. Specialty: Ambulatory Clinic / Medical Office Building. LVN Pay Rate (No Housing): $45/hr.

Duties and Responsibilities:

• Manage incoming telephone calls, email messages, patient portal requests, and Epic inbox tasks within the LVN scope of practice
• Review patient messages, gather the necessary clinical information, document communications accurately, and route concerns to the appropriate provider or licensed clinical team member
• Use approved SmartPhrases, message-management tools, and escalation procedures when responding to or routing patient concerns
• Recognize urgent or potentially serious patient concerns and promptly escalate them according to established clinic protocols
• Communicate provider instructions, appointment information, follow-up recommendations, and approved patient education
• Support physicians, RNs, and other members of the clinic team with daily patient-care activities
• Assist with patient intake, rooming, vital signs, health-history updates, and documentation of current patient concerns
• Prepare patients, examination rooms, supplies, and equipment for scheduled visits and procedures
• Perform routine nursing procedures within the LVN scope of practice and according to provider instructions
• Assist with dressing changes, wound care, specimen collection, ear irrigation, catheter care, and other approved clinical procedures when assigned
• Administer medications, vaccines, and ordered treatments within the LVN scope of practice and facility guidelines
• Reinforce standardized patient education, self-care instructions, and follow-up plans as directed by an RN or provider
• Review medical records for incomplete information and communicate missing or required information to the appropriate staff
• Document patient care, telephone encounters, messages, treatments, and follow-up actions accurately in Epic
• Coordinate care and communication among patients, providers, nurses, pharmacies, and other departments
• Follow infection-control practices, universal precautions, patient-safety standards, and department policies
• Recognize and respond appropriately to emergency situations according to established procedures
• Assist with maintaining efficient patient flow and timely completion of clinical and administrative tasks
• Participate in department quality-assurance and performance-improvement activities
• Perform other clinic-related LVN duties as assigned

Required Experience:

• Ambulatory care or outpatient clinic experience
• Epic EMR experience
• Experience managing telephone or electronic patient messages is highly preferred
• Ability to manage multiple patient messages and clinical priorities efficiently
• Experience communicating with providers and escalating clinical concerns appropriately
• Strong clinical documentation, telephone communication, and organizational skills
• Familiarity with Epic SmartPhrases, inbox workflows, or similar message-management tools is preferred
