Make a Difference Without Leaving HomePrime Medical Services is seeking an experienced, compassionate, and highly organized Licensed Practical Nurse (LPN) to join our growing Chronic Care Management (CCM) team.This fully remote role provides an opportunity to make a meaningful impact on the lives of residents living in skilled nursing facilities through proactive care coordination, chronic disease management, and resident engagement.
