What You'll Do

? Assess patients' psychosocial, financial, and environmental needs

? Complete initial and ongoing social work evaluations

? Support care planning and discharge planning

? Connect patients and families with community resources

? Communicate changes and concerns with care team and physicians

? Document visits and maintain compliance with care standards

? Provide counseling and guidance to patients and caregivers

What You Need

? Master's or Doctoral Degree in Social Work (CSWE accredited)

? Active Social Worker license

? 1+ year of healthcare social work experience

? Strong communication and interpersonal skills

? Ability to travel within the coverage area (50%)

? Valid driver's license, auto insurance, and reliable transportation

? Ability to lift, push, or pull up to 50-100 lbs
