Role and Responsibilities

Ensure quality and safe delivery of home care services.

• Participates in development and implementation of client’s plan of care per home care agency policy and procedure, as appropriate.
• Participates in client case conferences according to L & W Legacy Home Care Agency policy and procedure, if requested.
• Information regarding client’s plan of care is submitted to L & W Legacy Home Care SUPERVISOR in a timely manner.

Implements current Certified Nurse Aide services.

· Service plan of care is discussed with L & W Legacy Home Care Supervisor on regular basis.

· Client assignments and reports are received from L & W Legacy Home Care Supervisor.

Qualifications and Education Requirements

-High school graduation or GED required.

-Certified Nurse Aide certification required as obtained through successful completion of an approved program

-Criminal history check conducted is required prior to being offered work with this agency

-CPR/First Aide

-TB Skin Test within (1) year. Chest x-ray if positive (may require Dr. clearance)

-One year-full-time experience in personal care or an institutional setting, such as a hospital or nursing home, OR shall have one year full-time experience within the last five years in direct client care in a home care agency setting; OR will consider an applicant with a recent certification as a Nurse’s Aide

-Exhibits sympathetic attitude towards caring for the elderly

-Displays maturity and ability to deal effectively with job demands

-Demonstrates ability to read, write, and carry out directions and has good verbal and written communication skills

-Evidence of maturity and ability to deal effectively with job demands

-Attends (8) hours of personal care oriented in services per year

-Participates in professional meetings when directed

-Ability to work closely supervised to ensure competence in providing client care

Physical Demands

-Must be able to read & write English

-Must be physically able to lift, bending and stand for long periods of time

-Must be able to handle stressful situations due to client’s medical conditions

-Must adhere to assigned and agreed work schedule

-Able to drive up to 50 + miles per day

-Must maintain professional appearance and good personal hygiene

Competency Testing:

Certified Nurse Aide is expected to pass competency examination with at least an 80% or better. The content of the competency evaluation of the Agency will include and is not limited to: (if applicable)
