Reports to: AdministratorFull Wage Range: $40.00 - $50.00Full job description:Complete nutritional initial, quarterly, annual and significant change reviews on residents according to federal and stateguidelines. Assist in coordination of nutrition care services with Dietary Supervisor. Complete nutritional reviews monthlyon high risk residents (significant weight loss/gain, pressure ulcer, hemodialysis, and tube fed).Essential DutiesAbility to meet all health, compliance, and competency requirements.Attends and participates in morning meetings/stand up to facilitate communications with the team.Assess nutritional needs, diet restrictions and current health plans in order to develop and implement dietary careplans and provides nutritional counseling as needed.Monitor food services operations to ensure conformance to nutritional, safety, sanitation and quality standards, aswell as state and federal regulations.Monitor food control systems such as food temperatures, portion control, preparation methods, garnishment andpresentation of food in order to ensure that food is prepared and presented in an acceptable manner.Inspect diet trays for conformance to physician’s diet orders prior to delivery.Consult with physicians and other health care personnel (Dialysis Dietitians, Nurse Practitioners, etc.) todetermine diet restrictions and nutritional needs of residents.Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professionalcolleagues.Summary of BenefitsWe are proud to offer our staff low monthly premium payments for employees and dependents will be as follows:Medical, Dental, Vision401(k) plan with employer matchGenerous paid time off for vacation accruing at 4 hours a month, 48 FT or 40 PT annually recurring sick day hours prorated based on hire date, and 7 paid Holidays each year.EOE/M/F/VETS/DISABLED – At South Creek Post Acute, it is our promise to personally touch lives every day.
