Registered Nurse Case Manager
The Registered Nurse Case Manager coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. This role provides ongoing support and expertise through comprehensive assessment, care planning, implementation, and overall evaluation of individual patient needs. The aim is to enhance the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning functions.
Key Responsibilities
- Conduct comprehensive assessments to determine individual patient needs.
- Develop and implement personalized care plans that align with patient preferences.
- Coordinate services across multiple disciplines to ensure cohesive patient management.
- Advocate for patients by addressing their holistic needs and ensuring effective resource utilization.
- Facilitate discharge planning and transition of care processes.
- Monitor and evaluate the outcomes of care plans and make adjustments as necessary.
Qualifications
- Valid RN license in California.
- Minimum of 2 years of recent RN Case Management experience, specifically in Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) settings.
- Experience with MCG guidelines and discharge planning across acute care settings.
- Strong collaboration skills with interdisciplinary teams (IDT).
- Current Basic Life Support (BLS) certification from the American Heart Association.
Schedule
This position is full-time with a Monday to Friday schedule from 8:30 AM to 5:00 PM.
