Stony Brook CPMP seeks a Case Manager to enhance patient management, maximize satisfaction, and promote cost effectiveness across the full care cycle. The role requires professionalism, organization, and the ability to implement and evaluate actions to meet health and human service needs. The position is based in Stony Brook, NY, with a Monday–Friday schedule from 8:30 AM to 5 PM.
