Registered Nurse (RN) - Case Management

Case Manager – Acute Care

Contract duration is from October 4, 2026, through February 27, 2027, with a weekend rotation required to meet departmental needs.

Ratios are 1:20 with no floating expectations.

Requirements:

• 2 years of acute care hospital case management experience performing care coordination and discharge planning.
• A current California state license is required; pending licenses accepted if licensed by start date.
• No more than 7 days of requested time off is allowed.
• First-time travelers are accepted with clinical screening.

Responsibilities:

• Coordinate patient care and discharge planning effectively.
• Manage patient caseloads and ensure optimal patient outcomes.
• Collaborate with interdisciplinary teams to promote patient health.
• Evaluate and implement effective care strategies.
• Provide support and resources to patients and families as needed.
