Assess patients’ psychosocial needs and coordinate referrals to appropriate facilities or community agencies. Provide crisis intervention and bereavement counseling as part of comprehensive patient care. Collaborate with the multidisciplinary team to support discharge planning, care coordination, and patient advocacy. Document assessments, plans, and interventions in the medical record per policy and standards. Requirements Ability to provide psychosocial assessments and crisis intervention in an inpatient medical setting. Experience rendering psychosocial services and making referrals to facilities or community resources. Strong communication skills and ability to collaborate with interdisciplinary teams. Eligible for a travel assignment in California and available to commit to a 13-week contract starting on 08/31/2026.
