Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care and safe discharges for adult rehab patients in an inpatient setting. In this role, you’ll lead individualized discharge planning, conduct utilization reviews, secure authorizations, and connect patients with community resources to support recovery and independence. You’ll collaborate closely with physicians, therapists, and nursing to align goals, remove barriers, and educate patients and families.
