Key Responsibilities

• Perform initial and ongoing physical therapy evaluations in accordance with APTA and state standards, developing individualized Plans of Care with short- and long-term goals.
• Implement and adjust Plans of Care based on patient progress, incorporating therapeutic exercise, neuromuscular re-education, physical modalities, gait training, and manual techniques.
• Evaluate patient mobility, strength, and functional level; develop appropriate home exercise programs and discharge instructions upon completion of care.
• Document patient progress and maintain accurate records in compliance with facility procedures and third-party payer requirements.
• Maintain open communication with therapy staff, department staff, and referral sources involved in patient care.
• Supervise physical therapy assistants in a professional and consistent manner as indicated.

Required Qualifications

• Minimum 1 year of physical therapy experience required.
• Outpatient physical therapy experience required.
• Active Illinois Physical Therapist License required.
• Active American Heart Association (AHA) BLS certification required.
• Current Skills Checklist pertinent to Physical Therapy (within the last year) required.
• 2 completed Supervisory Reference Forms with dates of employment listed required.

Preferred Experience / Skills

• Experience with Cerner EMR.
• Strong knowledge of anatomy, exercise physiology, biomechanics, kinesiology, neuroscience, and pharmacology.
