Recovery Case Manager - Housing & Relapse Support (Hybrid)

South Middlesex Opportunity Council seeks a Case Manager to provide case management services for participants in a program serving episodically homeless individuals with relapse histories. You will perform assessments, develop an Individual Service Plan within three days of entry, coordinate referrals, and maintain confidential client files while ensuring HUD requirements are met.
