Hybrid Complex Case Manager - Behavioral Health & Care Coordination

Alliance Health is seeking a Complex Case Manager to engage and support members with high complexity, including multiple chronic conditions, SMI, SUD, I/DD, mobility impairments, frequent ED/inpatient use, and social determinants of health. Periodic onsite visits with members in hospital or residential settings are required.
