Clinical Care Coordination Manager - FT Days

Job Summary The Clinical Care Manager has a system wide view of the hospital and is responsible for oversight 24/7 central coordination of patient admission, discharges, hospital throughput and employee staffing. The Manager collaborates and coordinates with multidisciplinary teams to facility a safe quality experience for patients through the continuum of care, and will serve as the primary liaison between nursing, service locations, support departments, other healthcare facilities and in partnership with physicians to ensure appropriate patient placement for effective, efficient patient-focused outcomes. The Manager is responsible for budget/labor management, payroll, patient/customer satisfaction, employee engagement. Will work collaboratively with all members of the healthcare team to improve throughput, length of stay (LOS), and readmission reduction performance of the organization. Responsibilities Coordination and Facilitation_25% Manages all aspects of Care Coordination plans include patient progression, barrier escalation and resolution, continuous process improvement initiatives, operations and training. Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met, and treatment plans are executed in the appropriate level of the continuum. Leadership_25% Provides leadership, direction, resources, training and education, evaluation and support to staff. Responsible for staff selection, retention, and development. Collaboration and Optimization_25% Collaborate with Nursing, Social Services, Environmental Services, Transportation, Physicians, and other disciplines to facilitate patient placement including admission, transfers and discharges. Assist Care Coordination leadership in identifying, monitoring, and evaluating department operations to ensure optimal efficiency and effectiveness. Compliance_25% Partner with Quality Leadership to conduct quality assurance reviews ( e.g chart reviews, utilization reviews) of patient charts to verify compliance with all governing regulations and quality metrics. Support training/education as needed to improve QA/QI Qualifications Bachelor's Degree in a health related field Master's Degree in a health related field Preferred Six (6) or more year’s job related experience including minimum three (3) years of Case management experience. One (1) year of supervisory experience Registered Nurse in State of Georgia Basic Life Support (BLS) Case Management Certification Preferred Core Competencies These competencies reflect the values and behaviors expected of all Grady team members, regardless of role.
