Position Summary The Life Coach II serves as a Case Manager, providing service coordination and direct support to individuals. This role ensures quality care, supports treatment planning, and helps individuals achieve their personal goals in community and home settings. Key Responsibilities Provide case management and service coordination for individuals served Support individuals in achieving treatment plan goals and independence Develop and contribute to Individual Support Plans and treatment planning Coordinate services with clinicians, providers, and community resources Conduct home visits and monitor service delivery and client well-being Facilitate team meetings and collaborate with guardians and care teams Provide direct support and skills training as needed (daily living, social, coping skills) Maintain accurate and timely documentation (service notes, reports, billing, tracking) Participate in on-call rotation and follow crisis protocols Use de-escalation and crisis intervention techniques when needed Advocate for individuals and support access to services Maintain confidentiality and ethical standards Participate in trainings and team meetings Perform other duties as assigned Minimum Qualifications High school diploma or GED required Minimum two (2) years of experience working with individuals with developmental disabilities or mental health conditions (Related education/training may substitute for experience) Valid Alaska driver's license with acceptable driving record Must have an insured vehicle Ability to pass required background checks Preferred Qualifications Experience in case management or service coordination Knowledge of behavioral health, treatment planning, and community resources Strong documentation and organizational skills Experience working with multidisciplinary teams Core Competencies Strong communication and relationship-building skills Ability to manage multiple priorities and problem-solve effectively Compassionate, patient, and professional approach Ability to maintain boundaries and ethical practice Willingness to learn and grow in a human services career Physical Requirements Frequent standing, walking, and travel between locations Ability to lift up to 25 lbs regularly and up to 50 lbs occasionally Ability to respond to crisis situations, including verbal or physical de-escalation Work Environment Work in home, community, and office settings Exp sure to varying environments and weather conditions May encounter behavioral or crisis situations Equal Employment Opportunity Assets, Inc. is an Equal Opportunity and Affirmative Action Employer.
