Job Description
Title : Acute Care Case Manager - RN

Schedule :
- Orientation: 20 hours
- Weekends: Coverage required as needed

Assignment Details :

Requirements :
- Minimum 1 year RN experience
- Prior Epic charting experience required
- BLS certification required

Roles & Responsibilities :
- Coordinate patient care and discharge planning to optimize patient outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Manage durable medical equipment and home health/hospice placement.
- Facilitate SNF/assisted living placement and authorization coordination.
- Conduct appeals/denials, continued stay reviews, and ensure medical necessity.
- Adhere to CMS, Joint Commission/core measures, and HIPAA regulations.

Compliance / Notes :
- Up to 6 days total or 3 consecutive auto-approved time off; larger requests require approval.
- Must have a vehicle for this assignment.
- Travelers may be required to float; refusal may impact assignment.
