Growing together.Optum has an immediate opening for a friendly, patient focused and detail oriented Associate Patient Care Coordinator to join our team. Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.Schedule: 40-hour work week Monday through Friday between the hours of 7:00 am to 7:00 pm. May float to other offices as needed. The schedule will be determined upon hire.Location: 185 Route 312, Suite 303, Brewster, NY 10509Primary Responsibilities:Greets patients and visitors in a prompt, courteous and helpful mannerVerifies insurance information, address and phone number of the patient, registers new patientsArrives patients on the computer for scheduled appointmentFollows the payment security policy and procedures according to Optum Medical guidelinesAnswers incoming and outgoing phone calls in a courteous and prompt manner; directs calls and takes messages, ability to make confirmation callsAbility to maintain a work queue list- complete any pending actions ie referrals, scheduling appointmentsSchedules patients for future appointmentsAssists therapists with paperwork needed for pre-certification of therapy visitsConfirms new patient appointments for the next dayPerforms clerical duties such as copying, faxing and filingContacts patient in case of no call/no showAbility to use an electronic health record (EPIC)Effectively executes time management by notifying the provider team of the patient's arrival, consistently reviewing the daily patient schedule, and notifying the team of any potential conflicts, which may include voicemails, emails, and online bookings.Helps ensure continuity of care and communication between Optum providers and other partners as appropriate.Completes clinic reports and administrative duties to ensure exceptional patient care.Executes working knowledge of HIPAA privacy laws by protecting patient information. The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment.
